The Association for Individual Development (AID) is a non-profit organization whose mission is to empower people with physical, developmental, intellectual, mental health challenges; those who have suffered a trauma; and those at risk, to enjoy lives of dignity and purpose. We are looking for a Case Manager Qualified Mental Health Professional Intake for Behavioral Health who demonstrates this mission and wants to work for an organization that makes a difference in the community.
Provision of assessment, counseling, skills training and case management services to a caseload of clients who are diagnosed with persistent mental illness. Services are provided in individual treatment settings
Complete Intake/IM+CANS as assigned and other required paperwork
Conduct new client intakes including IM+CANS and LOCUS Assessments for all new client intakes
Maintain prescreening/intake logs
Provide documentation of services in accordance with DHS and CARF
Advocate for and maintain rights of clients that are served
Assignment of clients to case managers’ caseloads in coordination with program manager(s)
Essential Job Responsibilities:
Work scheduled hours, including evening hours
Be flexible to meet client and program needs, including cross training in other programs as assigned by supervisor.
Complete clinical assessments that meet the requirements of program funders and applicable administrative rules
Complete all required documentation in a timely manner per agency policy.
Acquire and maintain required certifications and trainings.
Schedule new client intakes within the timeframe set by agency policy.
Complete IM+CANS assessment, required intake paperwork, and LOCUS assessment in a timely manner per AID program standards.
Must meet minimum service hour provision productivity standards (MRO) monthly
Check/respond/Log Intake Line calls as assigned per county
Check/respond/Log IRIS referrals as assigned per county
Provide case management services as medically necessary

What will you bring to the table?
Education: Masters’s degree in a human services field (required)
Experience: 1+ years of experience in behavioral health preferred
Physical: Tolerance of a variety of home/living conditions of clients
Equipment:
- Computer with working knowledge of Microsoft Windows
- Copy machine
- Telephone with voice mail system
Additional Job Responsibilities:
Drive self and clients in personal automobile
Maintain automobile liability insurance as defined by AID’s personnel policies
Maintain valid Illinois driver’s license
Meet/maintain driving record standards as defined by AID’s insurance carrier.
