Purpose of the Position Under general supervision, promotes, maintains, and improves individual and community health by assisting individuals and communities to adopt healthy behaviors and preventing the spread of HIV. Collect and analyze data to identify community needs prior to planning, implementing, monitoring, and evaluating programs designed to encourage healthy lifestyles, policies, and environments. Performs HIV testing, counseling, outreach, and linkage to care for high-risk residents in the community. Ensures implementation of timely disease intervention strategies to decrease the prevalence of HIV and STDs. Other duties as assigned. This is a grant-funded position under HIV Prevention. This position works to promote and protect the health and safety of the residents of Cobb and Douglas Counties.
Essential Functions (include but are not limited to)
- Assesses community health needs to focus HIV Prevention efforts on target populations
- Provides community outreach activities to relevant communities with a focus on mem who have sex with men, transgender women, and people of color communities
- Provides educational programs and participates in community health fairs to enhance public understanding of HIV/AIDS and biomedical prevention tools
