Choice Health at Home is seeking a compassionate and skilled Physical Therapist to provide individualized care that helps patients regain mobility, improve function, manage pain, and maintain independence in the comfort of their own homes.

Perform comprehensive patient evaluations and develop individualized treatment plans.

Implement therapeutic interventions to restore function, improve mobility, and reduce pain.

Monitor patient progress and modify treatment plans as needed to achieve optimal outcomes.

Collaborate with physicians, nurses, caregivers, and interdisciplinary team members.

Educate patients and families on treatment goals and home exercise programs.

Maintain timely, accurate documentation and comply with agency policies and regulatory requirements.

Follow infection control and patient safety standards.

Graduate of an accredited Physical Therapy program.

Current Physical Therapist license in the state of practice.

CPR certification required.

Valid driver's license and auto insurance.

Home Health experience preferred.
