Overview
We are seeking a dedicated and compassionate Registered Nurse to join our team for immediate home health visits in. Responsibilities
- Conduct thorough patient assessments, including vital signs, physical examinations, and health histories to determine care needs.
- Develop and implement individualized care plans tailored to each patient’s condition and goals.
- Manage post-acute care transitions, including discharge planning and coordination with healthcare providers.
- Deliver skilled nursing interventions such as medication management, wound care, and infection control in accordance with healthcare regulations.
- Utilize Electronic Health Records (EHR) systems to document patient interactions accurately and maintain detailed medical records.
- Collaborate closely with physicians, therapists, social workers, and other healthcare professionals to ensure seamless patient care.
- Educate patients and their families on health management strategies, medication adherence, and safety protocols to promote recovery and well-being.
Requirements
- Valid RN license in or eligible for licensure in the state.
- Strong knowledge of medical terminology, physiology, infection control policies (HIPAA compliance), and health regulation standards.
- Excellent patient assessment skills combined with the ability to manage complex cases effectively.
- Ability to communicate clearly with patients, families, and healthcare team members while demonstrating empathy and professionalism.
