Position Overview
Heal Home Health is seeking a dedicated and experienced Licensed Vocational Nurse (LVN) to join our growing team. The ideal candidate must have strong clinical experience in wound care management and catheter care, and be passionate about providing high-quality care to patients in a home health setting. This role requires excellent clinical skills, strong communication, and the ability to work independently while delivering compassionate, patient-centered care.
Key Responsibilities
- Provide skilled nursing care to patients in their homes under physician orders and care plans.
- Perform comprehensive wound assessments and provide advanced wound care treatments, including dressing changes and infection prevention.
- Manage catheter care, including insertion assistance (as applicable), maintenance, monitoring, and patient education.
- Monitor patient conditions and report changes to supervising RN or physician.
- Administer medications as prescribed.
- Educate patients and families on disease management, wound care, catheter care, and safety practices.
- Maintain accurate and timely clinical documentation.
- Follow infection control protocols and home health regulations.
- Collaborate with interdisciplinary healthcare teams to ensure optimal patient outcomes.
Required Qualifications
- Current California LVN license in good standing.
- Required: Proven experience in wound care and catheter care.
- Previous home health experience preferred.
- Strong assessment and clinical decision-making skills.
- Ability to work independently and manage time efficiently.
- Excellent communication and patient education skills.
- Valid driver’s license and reliable transportation.
Preferred Qualifications
- Experience with electronic health records (EHR).
- Knowledge of Medicare home health guidelines.
