For over 30 years, St. Mary's Center Skilled Nursing Facility (SNF) has delivered high-quality medical care and essential social services to people living with HIV/AIDS in Harlem and New York City. In 1997 our Adult Day Health Care (ADHC) opened. St. Mary's Center expanded to provide medical care and social support to people in the community living with HIV/AIDS. Today the program offers more than 60 types of support groups and services each week. We recognize that fun is an important part of health and we offer numerous recreational opportunities to make fun a part of wellness.
St. Mary’s Center is hiring for a part-time Social Worker (20 hours a week). Under the direction of the Program Director, he/she will assess and treat HIV/AIDS individuals with mental, emotional or substance abuse problems, including abuse of alcohol, tobacco and/or other drugs. He/She will provide individual and group therapy, crisis intervention, case management, client advocacy, prevention and education.
RESPONSIBILITIES:
- Maintain a case management caseload.
- Counsel clients individual and group sessions to assist them in dealing with substance abuse, mental and physical illness or physical abuse, and provide the appropriate documentation in charts.
- Participate in the interdisciplinary comprehensive care plan (CCP) process and weekly meetings.
- Monitor, evaluate and record client progress in charts with respect to treatment goals.
- Liaison with outside community, governmental and/or regulatory agencies.
- Responsible for planning and developing outreach efforts and maintaining relationships with community-based organizations and SROs.
- Responsible for developing and implementing peer training program and supervising peer and outreach workers.
- Participate in various SNP (AmidaCare) services, including QA and improvement projects and marketing and enrollment initiatives.
- Assist in enrollment process into ADHC program; screen for eligibility
- Conduct timely reports and assessments of clients’ care plans.
- Coordinate and complete all appropriate supportive referrals and services.
- Perform any other duties as assigned by Program Director.
QUALIFICATIONS:
- Masters Degree in Social Work
- 3+ years’ experience in working in a adult day health care setting
- Knowledge of NYSDOH AIDS Institute requirements and procedures
- Knowledge of HASA agency policies and procedures governing adult day health care /AIDS Services
- Experience counseling people living with AIDS
- Active NYS LMSW
- EMR experience
- Bilingual (Spanish) preferred
Pay: $35.71 - $39.56 per hour
