Position: Physical Therapist (PT) – Home Health
Location : 1820 NW Mulholland Dr .Roseburg, OR 97470
Contract Duration: 13 Weeks
Schedule: Monday–Friday, 8:00 AM – 5:00 PM (Most assignments are 5x8s; some western territory positions may follow a Baylor schedule of 4x10s, Friday–Monday.)
Setting: Home Health
Key Responsibilities
- Provide physical therapy services to adult home health patients in their residences.
- Conduct Start of Care (SOC) evaluations and follow-up visits.
- Develop, implement, and modify individualized treatment plans.
- Complete accurate and timely clinical documentation using HCHB EMR.
- Manage a daily caseload of approximately 5–10 patients (5–6 average visits per day).
- Meet weekly productivity expectations of 30 points, with point values assigned to visits, documentation, travel, and other activities.
Qualifications
- Minimum 1 year of recent adult Home Health Physical Therapy experience (required).
- First-time Home Health travelers are not accepted.
- Active Physical Therapist license (license must be in hand before submission unless pending for only a few days).
- Current BLS/CPR certification.
- Experience using HCHB EMR is preferred.
- Valid driver's license, reliable transportation, and agency-required automobile liability insurance.

- Coverage Area: Contra Costa County and Alameda County, CA.
- Must be willing to travel up to 75 miles from the home clinic.
