As our Weekend On-Call Hospice RN, you will be the reassuring voice in the night, the skilled hands at the bedside, and the compassionate presence families can count on. You’ll respond to urgent needs, provide expert symptom management, and guide families through difficult decisions—helping them feel supported, informed, and never alone. What You'll Do
- Respond promptly to weekend and after-hours calls, triaging patient needs and determining the appropriate level of care.
- Provide in-home visits for urgent symptom management, medication adjustments, and emotional support.
- Collaborate with the interdisciplinary hospice team to ensure seamless continuity of care.
- Document assessments, interventions, and communications accurately in our EMR system.
- Offer guidance and education to family members and caregivers on patient comfort and care techniques.
- Serve as a calm, confident resource during moments of crisis or change.
What You'll Bring
- Current RN license in the state of practice, in good standing.
- Minimum 2 years of nursing experience—hospice, palliative care, or home health strongly preferred.
- Strong clinical assessment skills and the ability to make sound decisions independently.
- Exceptional communication skills, with a compassionate and patient-centered approach.
- Comfort with technology, including EMR documentation and mobile communication tools.
- A reliable vehicle and valid driver’s license for home visits.
