We are seeking a dedicated and compassionate Physical Therapist to join our rehabilitation team in Port Charlotte, Florida. In this role, you will play a vital part in helping residents regain strength, mobility, and independence while delivering the highest standard of care in a supportive and collaborative environment.
Evaluate residents and develop individualized treatment goals and plans of care
Implement physical therapy treatment plans and complete all required documentation and record keeping
Apply therapeutic techniques including gait training, ambulation, muscle strengthening, coordination, endurance, mobility, posture control, sensory awareness, sitting/standing skills, balance, and orientation
Design, fit, and train residents in the use of adaptive devices, including orthotics and prosthetics
Select appropriate physical modalities based on individual diagnosis, needs, and functional capacity
Collaborate with the interdisciplinary rehabilitation team to ensure comprehensive, resident-centered care
Instruct residents in mobility techniques and monitor progress, behavior, and rehabilitative potential
Modify treatment programs as needed to meet each resident’s evolving needs
Attend and contribute to care plan meetings, staffing conferences, and related meetings
Supervise supportive personnel in accordance with state regulations and professional standards
Participate in staff education and training related to physical therapy objectives and techniques
Order necessary supplies and equipment
Maintain, clean, and report repair needs for therapy equipment
Ensure residents and families receive care in a respectful, compassionate environment that upholds individual rights and dignity
