Responsibilities
- Provide skilled home health PT evaluation and treatment
- Develop individualized plans of care to promote safety, independence, and functional mobility
- Communicate effectively with patients, caregivers, and interdisciplinary teams
- Complete documentation in a timely and compliant manner
Requirements
- Current California PT License
- Home Health experience preferred (training/support available)
- Reliable transportation and commitment to patient-centered care
- Strong communication and time management skills
