SUMMARY
To provide transportation for patients to and from facility in accordance with current federal, state and local standards governing the facility, to ensure that the highest degree of quality care is maintained at all times.
ESSENTIAL JOB DUTIES AND RESPONSIBILITIES
- Transport patients to and from facility. Maintain complete and accurate log
- Operate vehicle in accordance with applicable traffic laws, Company Vehicle Policy, state and federal regulations
- Follow guidelines for vehicle maintenance and reporting vehicle malfunctions, traffic accidents, etc.
- Maintains cleanliness of vehicle – both interior and exterior
- Maintains proper forms in vehicles
- Understand, comply with and promote all rules regarding residents' rights; promote positive relationships with residents, visitors, and regulators, to include presenting a professional appearance and attitude
- Immediately report any hazardous conditions, equipment, accidents and/or incidents
- Attend and participate in training, educational activities and staff meetings; assist in orientation and training other staff
CODE OF CONDUCT
Must adhere to the Company’s Code of Conduct and Business Ethics policy including documentation and reporting responsibilities
QUALIFICATIONS
- Must have current Driver’s License with no driving violations on record
- Must have a high school diploma or equivalent
- Must have ability to understand and apply training, and in-service education
- Must hold and maintain a current CPR certification
- Must be capable of maintaining regular attendance

- Must have ability to read, write and follow oral and written directions at a level necessary to accomplish the job
- Must be able to maintain confidentiality regarding patient and company proprietary information
Must be able to relate positively and favorably to residents, families and other employees at all levels
Physical and Sensory. Requirements (With or Without the Aide of Mechanical Devices ): Walking/mobility, reaching, bending, lifting, transferring and repositioning residents, grasping, pushing and pulling, fine hand coordination; ability to distinguish smells, tastes and temperatures; ability to hear and respond to pages; ability to read and write; ability to understand and apply training and in-service education; ability to instruct other staff in training education and staff meetings; ability to communicate with residents and others; and ability to remain calm in emergency situations and when handling multiple tasks.
