Territory: We are now expanding our high-quality care throughout Palm Beach County, hiring skilled PTs to join our team. This position covers our Central Palm Beach Zone, including Wellington, Royal Palm Beach, West Palm Beach, and Lake Worth.

What You’ll Do:
Provide high-quality, one-on-one physical therapy to geriatric patients in their homes.
Complete Evaluations, Re-Assessments, and OASIS documentation (Start of Care, Resumption of Care, and Discharge).
Implement effective plans of care (POC) to improve patient mobility and safety.
Coordinate directly with our clinical director and as needed, the patient's physician.

What You Need:
Active Florida Physical Therapist (PT) License.
Current CPR/BLS Certification.
Valid Driver’s License and reliable transportation.
Home Health experience preferred (Kinnser/WellSky and OASIS experience is a plus).
