POSITION SUMMARY
The PRN Registered Nurse (RN) is responsible for conducting initial nursing assessments as needed and performing client reassessments as required to ensure safe, appropriate, and compliant care delivery. The RN must demonstrate strong clinical judgment, excellent documentation skills, and the ability to work independently in the field.
Responsibilities
Conduct comprehensive initial nursing assessments as assigned
- Perform ongoing client reassessments as needed
- Develop and update plans of care in collaboration with agency standards and physician orders (if applicable)
- Accurately complete all required assessment forms and clinical documentation
- Submit completed documentation to the office in a timely manner
- Communicate changes in client condition to appropriate parties
- Ensure compliance with Virginia home care regulations and agency policies
Travel to client homes within the assigned service areas

Qualifications
Current, active Virginia Registered Nurse (RN) license in good standing
- Minimum 1 year of nursing experience preferred
- Home care experience preferred
- Experience with WellSky (or similar EMR system) is a plus
- Strong assessment, documentation, and organizational skills
- Ability to work independently
- Reliable transportation and valid driver’s license
- Willingness to travel to Virginia Beach, Norfolk, Chesapeake
- Must have at least two years of nursing experience, including a minimum of one year in a supervisory or teaching role.
- Knowledge of home care principles, practices, and procedures.
- Valid driver’s license and proof of current PPD vaccination.
Familiarity with HIPAA regulations.

WORK ENVIRONMENT
Work occurs in diverse home settings and senior care facilities, requiring the ability to lift up to 25 pounds, walk, and climb stairs. Frequent independent travel within the service area of North Point Homecare is necessary.
