Travel RN Case Manager – Paramount, California | LTAC | Day Shift | 13-Week Contract
We are currently hiring an experienced Travel RN Case Manager for an exciting 13-week travel assignment at a leading LTAC hospital in Paramount, California. If you are a skilled Registered Nurse (RN) Case Manager with experience in care coordination, discharge planning, utilization review, and acute care case management, this opportunity may be a great fit for you.

Key Responsibilities:
- Coordinate and manage patient care plans to ensure quality outcomes
- Perform discharge planning, care coordination, and utilization review
- Collaborate with interdisciplinary teams (IDT collaboration) for patient-centered care
- Conduct comprehensive patient assessments and care evaluations
- Ensure compliance with healthcare standards and payer requirements
- Advocate for patients through proper resource utilization and continuity of care
- Improve patient satisfaction and cost-effective care management
Requirements:
- Active California RN License (Required)
- BLS Certification through American Heart Association (AHA)
- Minimum 5 years of recent RN Case Management experience
- Experience in LTAC, STACH, Acute Care, or Hospital Case Management settings
- Strong knowledge of MCG Guidelines, discharge planning, utilization review, and care coordination
- Excellent communication and interdisciplinary collaboration skills
Preferred Experience:
- Travel Nursing experience preferred
- Experience working in Long-Term Acute Care (LTAC) settings
- Familiarity with hospital EMR systems
