SUMMARY OF WORK PERFORMED:
Provide case management services for up t0 35 youth (case load will vary depending on census). Provide current and former foster youth transitional support services, including but not limited to, housing, education, employment, basic life skills and permanency. Case managers will be responsible for initial intake through exit processes with youth and will serve eligible youth ages 15-23 years old. Services provided will be in compliance with the ILS contract from Washington State DCYF as well as applicable housing contracts. ILS case managers work on behalf of youth to equip each youth with the skills to successfully transition to independence.
JOB DUTIES AND RESPONSIBILITIES:
1. Cultivate contacts and relationships with outside agencies and individuals that have both direct and indirect impact on the youth’s independence and transition. These include but are not limited to:
- State Social Workers
- Youth support system (Foster Families, Bio Families, etc.)
- Educational institutions
- Landlords
- State Regional Personnel
- Health and Mental Health Professionals
2. Initiate and complete all documentation including:
- Intake and exit forms
- Learning plans and Famlink Reporting
- Annual assessments
- Case manager contact records
- Quarterly reports
- All other documentation described in the ILS Contract or applicable housing contracts.
3. Meet with youth on a monthly basis as per the contract.
4. Conduct twice monthly ILS groups (minimum) that teach skills related to any of the life skill domains that youth are working to master.
5. Advocate on behalf of the youth with DCYF, educational institutions, employment agencies, landlords and other community service providers.
6. Regularly communicate with social workers, foster parents, biological parents and others in the youth’s support system.
7. Develop and maintain a supportive relationship to help guide youth from the foster care system to independence.
8. Participate in regular staffing, team meeting and staff meetings and trainings.
9. Participate in supporting the rotating after-hours phone on a regular and routine basis
10. Other duties as assigned.
REQUIRED QUALIFICATIONS:
1. Bachelor’s degree or Master’s degree from an accredited college or university in social services or similar field.
2. Proficient Microsoft Suite computer skills and Canva.
3. Previous experience working with adolescents or at-risk children and families.
4. First Aid/CPR/HIV certification.
5. Must have 2 hours of blood borne pathogens training in the past 5 years.
6. Valid driver’s license and acceptable driving record as required by Olive Crest’s insurance carrier.
7. Reliable vehicle and current proof of automobile insurance.
8. Criminal record clearance.
9. Ability to lift 25 pounds.
Preferred Qualifications
1. Preferred Child Welfare experience.
2. Preferred Case Management experience.
Pay: $24.00 - $26.00 per hour
