We’re seeking PRN Nurses to join our Health Center/AL
Primary Responsibilities:
· Assess and evaluates patient’s needs for, and responses to, care rendered
· Apply sound nursing judgment in patient care management decisions
· Provide primary and emergency care for occupational and non-occupational injuries and illnesses
· Administer over the counter and prescription medication as ordered
· Collaborate with the nursing team to create Plan of Care for all patients
· Coordinate self-driven wellness programming for those individuals who are more independent, and less reliant on assistance
· Direct and guide ancillary personnel and maintain standards or professional nursing
· Coordinate healthcare services (specifically those services that come from sources outside of the health center facility — like on-boarding and supporting hospice staff)
· Build and maintain relationships with other home health organizations
· Maintain consistent contact with local physicians to stay up-to-date on each individuals’ medical needs and service expectations
· Coordinate discharges for individuals whose needs are no longer being met
Qualifications:
· Valid LPN license required, will consider graduated nurses!
· Advance knowledge in EMR’s.
· Willingness to work flexible schedule and to fill in when needed.
· Excellent bedside manner and communication skills.
· Working knowledge of health care laws and regulations.
