Job Duties:
Conduct nutritional assessments and develop individualized care plans
Monitor weight changes and manage tube feedings
Complete MDS and CAA documentation and participate in discharge planning
Collaborate with nursing, therapy, and administration teams
Provide diet counseling and attend resident council meetings
Conduct audits, supervise dietetic technicians, and deliver staff training
Revise menus and ensure dietary compliance with facility standards
Qualifications:
Bachelor’s degree in Dietetics
Registered with the Commission on Dietetic Registration
Licensed by the Ohio Board of Dietetics
Minimum 2 years’ experience in geriatric care
Strong knowledge of long-term care regulations and MDS process
Excellent communication, computer, and decision-making skills
Ability to travel between facilities and perform inspections
