Responsibilities
- Conduct comprehensive patient assessments, including vital signs, medical histories, and physical examinations to determine treatment needs.
- Administer medications and injections accurately following prescribed protocols, including IV infusions and other specialized treatments.
- Monitor patient responses to medications and interventions, adjusting care plans as necessary to ensure safety and efficacy.
- Document all patient interactions, assessments, treatments, and progress meticulously within electronic health record (EHR) systems such as Epic or Cerner.
- Collaborate with multidisciplinary teams—including physicians, therapists, and case managers—to develop and implement individualized care plans tailored to each client’s needs.
- Educate patients on medication regimens, health coaching strategies, relapse prevention techniques, and wellness practices.
- Maintain compliance with HIPAA regulations and infection control standards while ensuring a safe environment for staff and clients.
Qualifications
- Valid LVN license in good standing with the state licensing board.
- Prior experience working with individuals with substance use disorders or behavioral health conditions preferred.
- Demonstrated proficiency with EMR/EHR systems such as Epic or Athenahealth; experience with medical documentation is essential.
- Knowledge of medication administration protocols including injections, IV infusions, and medication reconciliation.
- Strong understanding of physiology, anatomy, pharmacology, and diagnostic evaluation techniques relevant to addiction treatment.
- Experience in triage assessments or acute care settings is highly desirable.
- Excellent communication skills with the ability to work effectively within a multidisciplinary team while providing compassionate patient care.
