Full-Time Primary/Palliative Geriatric Nurse Practitioner
Office Location: Grants Pass (402 SE G ST) and/or Medford (606 Medford Center)
Work Schedule: Monday thru Friday, 8 to 5 flexible
JOB SUMMARY
The role of the mobile Nurse Practitioner at Riverside Home Healthcare and Hospice is an instrumental position supporting the organization’s mission of providing Life Changing Service. All NPs at Riverside are responsible for providing competent, cost-effective, timely and high-quality health care services that are within their scope of practice.
DUTIES & RESPONSIBILITIES
1. Deliver excellent clinical care in a variety of settings including; Skilled Nursing, Assisted Living, and Memory Care Facilities.
2. Maintain performance and productivity standards, operational efficiency, excellent customer service and best practice standards.
3. Treat acute and routine medical conditions via in person, telephone call or telehealth across all settings (e.g. SNF, ALF and house call visits).
4. Demonstrate critical thinking, expert care and best practice standards in the clinical decision making process.
5. Provide care for patients from initial history taking, clinical assessment, diagnosis, prescribing medications and treatments, providing education and evaluation of care.
6. Maintain compliance with the established standards of practice of medicine for Advanced Practice Nurse/Nurse Practitioner in the state of practice.
7. Document findings and actions appropriately in the patient’s medical record.
8. Maintain patient confidentiality at all times.
9. Participate in professional development activities and maintain professional affiliations and CME’s.
10. Maintain safe work environment for self, patient, family and staff by using appropriate PPE.
11. Work collaboratively with the Riverside team members and community partners such as ALF and SNF staff members, patient family members and home health and hospice team members.
12. Represent Riverside in a professional manner and strive to promote a cooperative atmosphere at all times by acting with tact and diplomacy and maintaining a positive attitude.
13. Maintain professionalism at all times by being dependable, autonomous and responsible.
14. Prioritize workload and utilize work time properly and productively.
15. Be humble and possess the ability to accept criticism and/or discipline.
JOB REQUIREMENTS:
1. Must have a Master’s degree from an accredited NP program, possess appropriate state licensure, prescriptive authorization, APN authorization and meet credentialing requirements.
2. Must be able to pass background check and drug test.
3. Maintain a current and valid driver’s license, have access to a reliable form of transportation and be willing to drive in various weather conditions.
4. Must be able to read, write and speak English fluently.
5. Ability to communicate effectively, in verbal and written form, with retail and medical partners at various levels, patients, family members, physicians and representatives of the community.
6. Sound understanding of all federal and state regulations including HIPAA and OSHA.
