Travel Hospice Registered Nurse (RN)
Location: De Pere, Wisconsin
Duration: 13 weeks
Schedule: Monday–Friday, 8:00 AM – 5:00 PM
Call: No on-call required
Job Overview
We are seeking a compassionate and experienced Hospice Registered Nurse (RN) to provide high-quality, patient-centred hospice care in the De Pere, Wisconsin area. The RN will manage a caseload of hospice patients, coordinate care with an interdisciplinary team, and support patients and their families throughout the end-of-life care journey.
Key Responsibilities
- Provide skilled nursing care to hospice patients in accordance with the individualized Plan of Care.
- Perform comprehensive initial and ongoing patient assessments.
- Develop, implement, and update care plans in collaboration with physicians and the interdisciplinary hospice team.
- Coordinate care with LPNs/LVNs, CNAs, social workers, chaplains, and other healthcare professionals.
- Educate patients, families, and caregivers on hospice philosophy, disease progression, symptom management, medications, oxygen therapy, and other care needs.
- Monitor pain and symptom management while ensuring patient comfort and dignity.
- Document all patient care accurately and timely using HCHB (Homecare Homebase).
- Communicate changes in patient condition to the attending physician and hospice physician.
- Maintain accurate clinical records and ensure compliance with Medicare, Medicaid, and regulatory standards.
- Conduct routine supervisory visits and monitor quality of care provided by the hospice team.
- Participate in quality improvement initiatives and interdisciplinary team meetings.
- Provide emotional support and bereavement resources to patients and families.
- Travel within a 60-mile radius to perform routine visits, PRN visits, change-in-condition visits, and death visits.
- Follow infection prevention protocols and safety standards.
Required Qualifications
- Active Registered Nurse (RN) license in the state of Wisconsin.
- Minimum 1 year of RN experience in hospice, home health, oncology, long-term care, or acute care.
- Current BLS certification through the American Heart Association (AHA).
- Experience with Homecare Homebase (HCHB) is required.
- Valid driver's license, reliable transportation, and current automobile insurance.
- Strong clinical assessment, communication, documentation, and case management skills.
Preferred Qualifications
- Previous hospice or home health case management experience.
- Knowledge of Medicare, Medicaid, hospice regulations, and end-of-life care.
- Excellent organizational and time management skills.
- Ability to work independently while collaborating effectively with an interdisciplinary team.
