Overview
Under the direct supervision of the Chief Physician & Surgeon, and the general supervision of the Administrator and/or Skilled Nursing Facility Administrator, the physician and surgeon shall assume complete responsibility as primary care provider for specifically assigned residents to provide complete and total medical care. The physician and surgeon shall commit to the highest standard of medical care and shall be personally responsible to pursue continuous medical education in order to accomplish this goal. Provider must be linked, through their NPI, to their facility with Medicare, MediCal / DentiCal, and all other applicable insurance vendors.
Duties
- Provide primary care for residents in the medical clinic through routine and regular medical evaluation of residents admitted to the Veterans of Home of California – Fresno. Perform complete physical examinations on an annual basis. Provide written documentation in the unit health record including physical findings, diagnosis and therapeutic treatment plan, prescribed medications, progress notes, problem list and discharge summary. Identify patient(s) requiring transfer to facilities for continued/higher level of health care. Order and review necessary laboratory and radiological examinations and respond to reports/results. Refer to specialists when medical care exceeds the ability and/or expertise of the physician and surgeon.
- Admit and follow with monthly medical rounds, or sooner as needed, in the Skilled Nursing Facility. Visit patients and provide documentation of orders, progress/decline of health status and documentation of preferred intensity of care. Provide overall guidance and instructions to staff involved with patient care for the residents.
- Provider will complete appropriate charge slips for all resident visits in order for the veteran’s home to be reimbursed for services provided. Prepare reports and correspondence and do other administrative functions as directed.
- Attend and participate in staff conferences and cases discussions of resident medical condition. Participate in the Interdisciplinary Team Meetings as needed. Make recommendation for improving level of care. Attend medical staff committee meetings and participates in clinical projects. Serve as member/chairman of various committees.
- Take after hours; weekend and holiday call on a rotational basis and share coverage of all duties for providers out sick or on leave. Must maintain a valid CPR certification.
Requirements
- Possession of the legal requirements for the practice of medicine in California as determined by the California Board of Medical Quality Assurance or the California Board of Osteopathic Examiners. (Applicants who are in the process of securing approval of their qualifications by the Board of Medical Quality Assurance or the Board of Osteopathic Examiners will be admitted to the examination, but the Board to which application is made must determine that all legal requirements have been met before candidates will be eligible for appointment.) and Completion of an internship in an approved hospital. (Applicants serving an internship will be admitted to the examination, but they may not be appointed until they meet the other requirements stated above.)
- Knowledge of: Methods and principles of general medicine and skill in their application; methods and principles of surgery and skill in their application; recent developments in general medicine and surgery; hospital organization and procedure; gross pathology and interpreting autopsy findings; psychiatric social work, physical therapy, and the various rehabilitation therapies; methods of diagnosing and treating elementary mental disorders; principles of effective supervision and directing the work of medical personnel.
- Ability to: Interpret autopsy findings; examine, diagnose, and treat physical and elementary mental disorders; interpret laboratory analyses and x-rays; direct the work of medical personnel; instruct in the principles and practices of general medicine and surgery; prepare and supervise the preparation of case histories and the keeping of hospital records; analyze situations accurately and adopt an effective course of action.
Pay: $20,746.00 - $27,430.00 per month
