Responsibilities:

Monitors and tracks changes in the client/patient's conditions and needs and keeps the Director of Nursing and any other appropriate personnel informed.

Proactively updates client/patients' family on plan of care, health changes, and concerns.
Requirements:
-
Current OT license
-
Prefer Two (1) years of clinical experience

Prefer one (1) year experience in a home health agency

Current Drive License and reliable transportation

Submit to a background check and drug test

Excellent interpersonal skills.

Excellent customer service skills and ability to deal tactfully with clients, staff and community.
