Position Overview:
We are seeking a compassionate and reliable Licensed Practical Nurse (LPN) to provide direct patient care in the home setting under the supervision of a Registered Nurse (RN). The LPN will deliver skilled nursing services, assist in implementing patient care plans, and help promote positive clinical outcomes for our home health patients.
Responsibilities:
Provide skilled nursing care to home health patients as directed by the supervising RN and physician orders
Assist with OASIS Start of Care, Resumption, Recertification, and Discharge visits as needed
Administer medications and treatments accurately and safely
Monitor and document patient condition, progress, and response to care
Communicate any changes in patient status to the RN Case Manager and physician promptly
Educate patients and families on care techniques, medications, and home safety
Maintain timely, accurate documentation in compliance with Medicare and agency standards
Collaborate with the interdisciplinary team to ensure quality and continuity of care
Active Florida LPN license (required)
Minimum 1 year of Home Health experience (Medicare-certified agency)
Current BLS certification
Strong clinical skills, attention to detail, and communication abilities
Reliable transportation, valid driver’s license, and current auto insurance
