We are currently seeking a compassionate and motivated Speech Therapist to join our growing clinical team.
Position Summary
As a Home Health Speech Therapist, you will evaluate and treat patients experiencing communication, cognitive, voice, language, and swallowing disorders. You will work closely with patients, caregivers, physicians, and interdisciplinary team members to develop individualized treatment plans that improve communication abilities, cognitive functioning, and overall quality of life.

Responsibilities
- Perform comprehensive speech-language pathology evaluations in the home setting
- Develop individualized treatment plans based on patient needs and physician orders
- Evaluate and treat speech, language, cognitive, voice, and swallowing disorders
- Provide evidence-based therapeutic interventions to improve patient outcomes
- Educate patients, family members, and caregivers regarding treatment plans and home programs
- Monitor patient progress and adjust treatment approaches as needed
- Complete accurate and timely clinical documentation
- Collaborate with physicians, nurses, therapists, and interdisciplinary team members to coordinate patient care
- Maintain compliance with agency policies and applicable state and federal regulations
Qualifications
Required
- Active California Speech-Language Pathologist License
- Master's Degree in Speech-Language Pathology from an accredited program
- Current CPR Certification
- Valid driver's license and reliable transportation
- Strong communication and interpersonal skills
- Ability to work independently and manage a flexible schedule
Preferred
- Certificate of Clinical Competence (CCC-SLP)
- Home Health experience
- Experience working with geriatric patients
- Familiarity with electronic medical record (EMR) systems
