Job Overview
We are seeking a passionate and dedicated Occupational Therapist (OT) to join our dynamic healthcare team. Responsibilities
- Conduct comprehensive occupational therapy assessments to evaluate patients’ functional abilities and needs
- Develop and implement individualized treatment plans aligned with clinical goals and patient-centered outcomes
- Assist patients with activities of daily living (ADLs) such as dressing, grooming, feeding, and mobility to promote independence
- Coordinate discharge planning and collaborate with multidisciplinary teams to ensure seamless transitions across care settings
- Manage electronic health records (EHR) accurately, documenting clinical notes, progress reports, and treatment outcomes
- Provide education and training to patients, families, and caregivers on therapy techniques and adaptive strategies
Skills
- Strong knowledge of anatomy, physiology, medical terminology, and clinical documentation standards
- Experience working with individuals with disabilities in skilled nursing facilities.
- Proficiency in occupational therapy assessments and treatment plan development for pediatrics, geriatrics, and adult populations
- Skilled in assisting with ADLs and implementing occupational therapy programs tailored to specific needs such as post-acute care or outpatient rehabilitation
- Familiarity with case management practices including discharge planning and client communication
- Ability to collaborate effectively within interprofessional teams including speech therapists and other healthcare professionals

Pay: $40.94 - $55.00 per hour
