DESCRIPTION & PURPOSE:
The Enhanced Care Management (ECM) Case Worker provides comprehensive case management services for clients with complex medical, behavioral, and social needs, helping them access care and navigate resources. This role emphasizes person-centered, holistic care, addressing medical, housing, social, and mental health needs to improve clients’ health outcomes, stability, and quality of life.

LOCATION: Supportive Outreach Services
SALARY RANGE: $21 - $25 an hour
EMPLOYMENT TYPE: Full-Time / Part-Time
RESPONSIBILITIES: Case Management and Support:
- Conduct comprehensive needs assessments, develop personalized care plans, and set achievable goals in collaboration with clients.
- Connect clients with healthcare services, housing, social services, and community resources, coordinating care across providers and systems.
- Provide ongoing support, motivation, and education to empower clients in achieving self-sufficiency and improved health outcomes.
Advocacy and Client Relations:
- Advocate for client needs across healthcare, housing, and social service providers, ensuring continuity of care and equitable access to services.
- Build trusting relationships with clients to support their journey towards stability, independence, and self-determination.
Documentation and Compliance:
- Maintain accurate, timely, and confidential client records, documenting all interactions, progress, and updates.
- Ensure compliance with organization policies, regulatory requirements, and grant conditions, following up on outcomes and reporting data as required.
Collaboration and Coordination:
- Work closely with internal teams, Managed Care Plans (MCPs), community organizations, and healthcare providers to optimize client care and service integration.
- Attend regular team meetings, case conferences, and professional development training to stay current with ECM standards and best practices.
Crisis Intervention and Problem Solving:
- Provide immediate support and intervention during crisis situations, working with clients to resolve issues and prevent future crises.
- Identify barriers to care for and assist clients in problem-solving to maintain progress on care plan goals.
Additional Responsibilities as assigned, including but not limited to:
ECM Care Managers may be asked to provide Community Health Worker (CHW) services under an addendum. CHW training will be provided.
QUALIFICATIONS:
Education:
- Bachelor’s degree in social work, psychology, counseling, public health, or a related field.
- OR 2 years of relevant experience can substitute for the degree.
- Experience: 2+ years of case management, social work, or relevant experience in healthcare, housing, or social services, preferably with high-need populations.
Skills:
- Strong interpersonal and communication skills, with the ability to engage clients from diverse backgrounds.
- Knowledge of community resources, healthcare services, housing supports, and benefits programs.
- Proficient in case management software and basic computer applications (Microsoft Office Suite).
- Ability to work both independently and collaboratively in a team-oriented environment.
- Crisis intervention and conflict resolution skills.
Additional Requirements:
- Valid driver’s license and reliable transportation.
- Willingness to travel locally for client home visits, meetings, and events.
- Ability to pass background checks and maintain confidentiality.
Preferred Competencies:
- Experience working with managed care plans (MCPs) or CalAIM initiatives.
- Lived experience or demonstrated empathy with underserved populations, including individuals facing homelessness, behavioral health challenges, or complex medical conditions.
Working Conditions:
- Office and field-based position with regular local travel.
- Flexibility to work occasional evenings or weekends as needed for client support or program events.
Pay: $21.00 - $25.00 per hour
