Job Summary
This position functions as a Caregiver role, which is essential to maintaining uninterrupted services for participants enrolled in the Medicaid Home and Community Based (HCB) Waiver Program.
Key Duties and Responsibilities
- Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and feeding.
- Prepare and serve nutritious meals according to dietary needs and preferences.
- Provide companionship and emotional support, promoting a positive and respectful environment.
- Support client mobility and transportation, including assistance with walking, transfers, and attending appointments or outings.
- Monitor client condition and promptly report changes or concerns to supervisors or assigned case managers.
- Perform light housekeeping, laundry, and home organization to maintain a clean, safe living environment.
- Follow and help implement goals outlined in the individualized Plan of Care.
- Encourage client independence and participation in daily routines and activities.
- Maintain accurate records of services provided and complete documentation per program requirements.
Qualifications
- Prior caregiving experience in home care, long-term care, or group home settings is preferred.
- Familiarity with Medicaid Waiver Programs or experience working with individuals with disabilities is a plus.
- Basic understanding of medication reminders, safety protocols, and first aid.
- Ability to prepare simple, healthy meals tailored to client needs.
- Compassionate, dependable, and committed to preserving client dignity and autonomy.
- Strong communication and interpersonal skills.
- Must be able to work independently and manage time effectively.
- Valid driver’s license and reliable transportation preferred for client-related travel.
- CPR/First Aid certification preferred; training will be provided.
- TB Test preferred; will be provided.
