About the Opportunity
Octo Search Partners is partnering with Elevated Community Health (ECH) to identify a Registered Nurse for its primary care clinic in Frisco, Colorado. ECH is a nationally recognized Federally Qualified Health Center (FQHC) serving Summit County with integrated medical, dental, and behavioral health services delivered through a collaborative, team-based model of care. This role sits at the center of the clinic's care team, supporting a diverse patient population in one of Colorado's most sought-after mountain communities, with Breckenridge, Copper Mountain, Keystone, and Arapahoe Basin all nearby.
This role offers a true primary care nursing experience, blending hands-on clinical work with care coordination and patient education. The right candidate will value both sides of that mix and enjoy building ongoing relationships with patients over time. The role reports to the Medical Director.
What You Will Do
- Conduct independent nurse visits, including diabetes education and wound care, with room to expand nurse-led services as the team grows
- Provide case management for high-risk and medically complex patients navigating the healthcare system
- Deliver high-quality patient triage by phone and in person
- Provide direct patient care during nurse visits and co-visits with medical providers
- Serve as a clinical resource for medical assistants, front desk staff, health educators, and providers
- Coordinate care with pharmacies and payers, including prior authorization support and prescription refills per protocol
- Document accurately and timely in the electronic health record, including labs, vitals, and patient communications
- Participate in the clinic's QI/QA program and work as an active member of the integrated care team alongside Dental and Behavioral Health
Who We Are Looking For
- Current Colorado RN licensure and current CPR certification
- BSN preferred; associate degree candidates considered
- At least two years of clinical experience preferred; new graduates considered on a case-by-case basis
- Mission-driven nurses who genuinely want to serve an underserved, culturally diverse patient population
- Someone who understands and embraces the primary care nursing model: a mix of hands-on patient care and phone-based care coordination
- Bilingual Spanish skills strongly preferred but not required
- A team player who thrives in a fast-paced, collaborative clinic environment
