We are seeking a mission-driven Case Manager to join our team in a professional office environment. In this role, you aren’t just managing a caseload; you are a vital advocate using a "strength-based" approach to assess, plan, and treat youth and their families during their most critical moments.
Your Impact: Advocacy & Reintegration
As a Case Manager, you are the central link between our youth and the services they need to succeed. Your leadership ensures every child receives the care required for community reintegration. Key responsibilities include:
- Collaborative Planning: Developing individualized written plans of care and treatment.
- Court Representation: Managing services in cooperation with the courts and attending all hearings.
- Safety & Coordination: Coordinating clinical and drug testing, while ensuring all services meet the client’s specific needs.
- 24/7 Dedication: Providing 24-hour, 7-day-a-week coverage for case management to ensure our youth are never without support.
Community Advocacy: Representing our programs through public speaking and community committees.
The Profile: What You Bring
We are looking for a professional who manifests compassion, justice, and respect for the dignity of others.
- Education: Bachelor’s degree in a human service field or related degree.
- Experience: Minimum of two years of work experience in a child-caring or child-placing institution.
- Expertise: Extensive knowledge of systems of care, adolescent development, and criminogenic needs.
- Skills: Advanced time management, conflict resolution, and the ability to thrive in a high-paced, sometimes stressful environment.
Commitment & Requirements
- Schedule: Typically, Monday through Friday, 9:00 AM to 5:00 PM, with 24/7 on-call coverage responsibilities.
- Licensure: Must possess a valid, unrestricted Michigan driver’s license and reliable transportation for occasional travel to partner locations.
- Clearances: Must obtain satisfactory DHS Central Registry clearance, criminal background check, negative TB test, and drug screen.
- Physical Demands: Ability to perform light work, including moving objects up to 20 pounds and navigating stairs.
