Johnson County Care Center is a 87 bed nursing facility located in Warrensburg, MO. We offer excellent 24/7 care for our residents. Our facility is ICF certified, therefore we have limited heavy care and no skilled units. We follow the CDC guidelines for the COVID virus and remain COVID FREE!
Job Description
- Review the Communication Book daily and Care Plan behaviors etc.
- Run report daily and enter all entries and discharges,
- Open all MDS, Care Plans and Assessments Monthly and send out MDS Calendar to team prior to the last week of the month
- Have all diagnosis on new admissions into the PCC program within 24 hours, If they admit on Friday, these need to be entered on Monday.
- Have a base line care plan in the PCC program within 24 hours of admission
- Monitor daily for all new antibiotics and make sure these are care planned and diagnosis is updated under diagnosis.
- Check daily for all new antianxiety, depression and psych meds and make sure they have the appropriate diagnosis and update the diagnosis in the proper program. If they do not have the appropriate diagnosis, work with nursing dept. to get the appropriate diagnosis or medication is changed.
- Attend Care Plan meetings and ensure the participation sheets are filled out and the comprehensive care plan is brought to the meetings and Social Services ensures the resident/family is signing and returned back to MDS Coordinator to uploaded into the resident’s documents
- Ensure diagnosis list is updated prior to doing each quarterly or annual MDS
- Ensure all teams are doing their parts of the MDS and Care Plans in a timely manner
- Complete DA124’s if not completed or, verify this has been done upon admission and complete once the admission MDS is completed (this should be day 7 or 8 after they admit)
- Complete Level 2 screenings and submit. follow the process through until Medicaid Approval is completed.
- Update and keep updated the 672 and 802 reports
- Ensure all wound care is documented in the care plan to match the wound report
- Every other Week Call.
- Be available during state survey to assist with gathering documentation needed by survey team.
- Nursing Administration responsibilities as assigned
