Job Overview
We are seeking a compassionate and dependable Registered Nurse to join our Adult Day Health Center team. In this role, you will provide nursing oversight, health monitoring, care coordination, and support for older adults and adults with medical, cognitive, and functional needs in a community-based day program. The RN will work closely with the Program Director, Care Coordinator, Activity Director, Program Aides, families, physicians, and case managers to ensure participants receive safe, appropriate, and person-centered care.
Bilingual or multilingual candidates are strongly encouraged to apply. Cape Verdean Creole, Portuguese and Spanish language skills are highly valued in this role due to the community we serve.
Responsibilities
- Conduct nursing assessments, review health histories, monitor participant needs, and assist with individualized care planning.
- Monitor vital signs, weight, blood glucose readings, pain, and other health indicators as ordered or clinically indicated.
- Review physician orders, medication lists, diagnoses, and medical documentation to support safe admission and ongoing participation.
- Administer medications and treatments as ordered and within RN scope of practice and center policy.
- Identify and respond to changes in participant condition, including illness, falls, behavioral changes, abnormal vital signs, or other health concerns.
- Communicate with physicians, families, caregivers, case managers, pharmacies, and other healthcare professionals as needed.
- Participate in the development, review, and updating of individualized care plans.
- Provide nursing support for participants with chronic conditions, dementia or memory care needs, diabetes, hypertension, mobility limitations, behavioral health needs, and other medical concerns.
- Document nursing notes, assessments, incidents, interventions, communications, and participant updates accurately and timely.
- Support infection control practices, health screenings, emergency preparedness, and participant safety procedures.
- Provide participant and family education related to medications, chronic disease management, nutrition, fall prevention, infection control, and overall wellness.
- Collaborate with the interdisciplinary team to ensure participants receive safe and appropriate care throughout the day.
- Assist with staff education and guidance related to participant care needs, safety protocols, documentation, and infection control.
- Maintain confidentiality and uphold HIPAA standards at all times.
Requirements
- Valid Registered Nurse license in the State of Rhode Island.
- Current CPR/BLS certification.
- Electronic health record experience is required. Candidate must be comfortable using digital documentation systems accurately and consistently.
- Strong computer skills are required, including the ability to use Office 365, Microsoft Teams, email, digital forms, and other digital communication tools.
- Must be comfortable working in a fully digital environment with electronic documentation, team communication, scheduling, and follow-up processes.
- Experience working with older adults, adults with disabilities, dementia/memory care, long-term care, home care, assisted living, adult day health, or community health preferred.
- Strong nursing assessment, documentation, communication, and care coordination skills.
- Bilingual or multilingual skills strongly preferred, especially Spanish, Cape Verdean Creole, or Portuguese.
- Must be dependable, compassionate, organized, and able to work well in a team environment.
