Duties
- Provide personalized assistance with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patient health by observing vital signs and recognizing changes in condition; accurately document medical information.
- Assist with medication administration following prescribed schedules and medical terminology guidelines; ensure proper storage and documentation
- Support with complex needs such as catheter care, ALS care, and end-of-life care with sensitivity and professionalism
- Facilitate safe patient transfers using methods such as patient lifts or Hoyer lifts; employ patient transfer techniques that prioritize safety for both caregiver and client
- Maintain patient confidentiality in accordance with HIPAA regulations; document medical records meticulously and adhere to home health care regulations
- Perform light housekeeping tasks including cleaning, laundry, formula meal prep , and ensuring a safe living environment for him.
Requirements are a plus but not required with paid training .
- Proven experience working with individuals with disabilities, geriatrics, or in assisted living environments; prior home healthcare or nursing home experience is highly valued
- Strong knowledge of medical terminology, vital signs measurement, medication administration, and basic life support (BLS) or CPR certification
- Ability to perform heavy lifting when necessary and operate patient transfer equipment such as Hoyer lifts safely
- Familiarity with EMR/EHR systems for accurate documentation; understanding of home health agency procedures and hospice or palliative care practices preferred
- Excellent communication skills for engaging effectively with clients, families, and healthcare professionals
- Valid certifications in first aid and CPR; knowledge of home & community care standards is advantageous
- Ability to work independently in a live-in setting while maintaining professional boundaries and providing compassionate patient care
