DUTIES
Our talented Certified Home Health Aide (CHHA) and Certified Nursing Assistant (CNA) will provides personal care and related services in the home of our wonderful client, under the direction, instruction and supervision of the staff nurse and the Nurse Supervisor.
This position need to be base in East Boston/Everett Area. However, our clients are base in the East Boston, MA, Everett, MA, Medford, MA Malden, MA and Revere, MA (Northshore) area. You must be a Spanish Speaking Bilingual (Spanish/English)
Tasks to be performed by an HHA must be assigned by, and performed under, the supervision of an RN, who will be responsible for the patient care provided by the HHA.
RESPONSIBILITIES
- Follows the plan of care to provide, safe, competent care to the client.
- Helps the patient maintain good personal hygiene and assists in maintaining a healthful, safe environment.
- Plans and prepares nutritious meals, markets, when instructed to do so by the nurse.
- Assists the patient with ambulation, as ordered by the physician, and approved and supervised by the nurse.
- Assists the therapy personnel, as needed, with rehabilitative processes.
- Encourages the patient to become as independent as possible, according to the nursing care plan.
- Attempts to promote patient's mental alertness, through involvement in activities of interest.
- Gives simple emotional, and psychological, support to the patient, and other members of the household, and establishes a relationship with patient and family, which transmit trust and confidentiality.
- Reports any change in the patient's mental, or physical, condition, or in the home situation, to the staff nurse, or to the Aide supervisor.
- Performs routine housekeeping tasks, as related to a safe and comfortable environment for the patient, as instructed by the professional nurse.
- Prepares a visit report, promptly, and incorporates same in the clinical record, weekly.
- Confirms, on a weekly basis, the scheduling of visits so that other necessary visits, by staff members, can be coordinated.
- Works with personnel of other community agencies involved in the patient's care, as directed by the nurse.
- Attends in-service, as required by regulation.
- Monitor patients (vital signs, temperature, respiration, etc) and report on their condition.
- Maintain patients care records and document provided services.
QUALIFICATIONS
1. Must hold a current Certified Home Health Aide (CHHA) certification; a high school diploma or higher is preferred
2. Bilingual: Spanish/English written and verbal communication and organizational skills.
3. Must possess a valid driver's license and reliable personal vehicle, with the ability to travel to client locations throughout Charlestown, Somerville, Malden, Everett, Chelsea, Revere, and East Boston (North Shore)
4. Must have successfully completed one of the following:
- A minimum 75-hour training and competency evaluation program as specified in 42 CFR 484.80(b) and (c); or
- A competency evaluation program meeting the requirements of 42 CFR 484.80(c); or
- A nurse aide training and competency evaluation program approved by the Commonwealth of Massachusetts meeting the requirements of 42 CFR 483.151–483.154, with current good standing on the Massachusetts State Nurse Aide Registry; or
- The requirements of a Massachusetts state licensure program meeting the provisions of 42 CFR 484.80(b) and (c)
5. Must be free from health conditions that could be injurious to patients, self, or co-workers, and must provide appropriate documentation to substantiate this
6. Proven experience working as a Home Health Aide
7. Strong understanding of personal care, housekeeping, and meal preparation fundamentals
8. Ability to monitor vital signs and collect specimens
9. Familiarity with basic nutrition and personal hygiene standards
10. Knowledge of nursing and healthcare administration practices
11. Caring, compassionate, and patient-centered personality
12. Must understand and respect client rights, including ethics and confidentiality of care
13. Must hold a current CPR certification and First Aid training — online-only certifications are not accepted
14. Must be able to successfully pass a criminal background check
JOB CONDITIONS
- Must be able to drive and access client homes, which may not be wheelchair accessible
- Must be able to communicate clearly, both verbally and in writing, in Spanish/English
- Hearing, eyesight, and physical dexterity must be sufficient to perform physical assessments and deliver client care
- Must be able to occasionally bend, stoop, reach, and assist with moving clients weighing up to 250 pounds; must be able to lift and/or carry up to 30 pounds
EQUIPMENT OPERATION
Blood pressure cuff, thermometer, stethoscope, and standard hand hygiene materials.
COMPANY INFORMATION
Has access to all client medical records, which may be discussed with the Registered Nurse and the Nurse Supervisor.
