Summerfield Post Acute Rehab is a preferred provider of POST ACUTE REHAB services in Santa Rosa, CA. We have a solid reputation in the community with a long history of success and we need the best of the best to join us on our pursuit of excellence. We are currently looking for an enthusiastic and motivated Administrative Assistant to assist our Social Services Director.
Essential Functions:
- Dishcharge Planning- Including scheduling of doctors appointments and arranging Home Healthcare, transportation and ordering of medical equipment as needed.
- Communicate with Physicians regarding medical care and discharging planning.
- Interview residents/families as necessary.
- Coordinate and assist Social Services Director with Family Care Conferences with residents and resident family members in regards to their care and discharge.
- Make appointments for residents and arrange transport to and from the facility.
- Team player with a positive attitude!
The ideal candidate will have prior experience in the following:
- Healthcare (minimum 1-2 years)
- Understanding of medical terminology
- Experience with outside patient care services such as; transport, in home care, hospice, etc.
- Discharge planning
- Experience with insurance coverage such as; Medical, Medicare, Partnership, etc.

Compensation :
Competitive depending on experience.
