The Hospice Care Specialist manages and facilitates the administrative and logistical aspects of patient care throughout the hospice journey. This role serves as the central point of contact for patients, families, and the interdisciplinary team (IDT), ensuring that all patient needs are addressed efficiently while supporting the clinical staff in providing high-quality hospice care.
Conduct initial hospice intake processes and documentation.
Verify patient eligibility for hospice services, including insurance and benefit coverage.
Collect patient information, medical history, and relevant documentation.
Coordinate with nurses and physicians to ensure smooth admission.
Track ongoing patient needs and communicate updates to the IDT.
Facilitate the ordering of medications, medical supplies, and durable medical equipment (DME) as requested by licensed staff.
Assist with scheduling visits, transfers, and changes in level of care.
Serve as the primary administrative contact for patients, families, and external providers.
Relay patient concerns, needs, and updates to nurses, physicians, and the IDT.
Document all communications and care coordination activities in the patient record system.
Maintain accurate patient records, ensuring compliance with regulatory standards.
Track and report on care-related tasks, supply orders, and patient transitions.
Support the hospice team in operational and compliance tasks as needed.
Experience in healthcare administration, care coordination, or customer service is highly desirable.
Strong organizational, communication, and problem-solving skills.
Knowledge of hospice regulations and processes a plus.
