Summary
Provide diagnostic, therapeutic and consultative services for individuals with speech, language and swallowing disorders as referred within Titusville Area Hospital and within the regional counties serviced by TAH.
Essential Functions
- Notify supervisor and/or physician of observations, changes and reactions regarding patients.
- Documents on patient record observations, changes and reactions and the reporting of them to the appropriate person (physician/supervisor, etc.)
- Communicates with nursing regarding swallowing guidelines and posts recommendations.
- Documents on patient record the health teaching to the medical staff, patient and/or significant other.
- Provides information on patient disorder(s) and therapeutic goals to patient and/or significant other.
- Demonstrates expertise in family dynamics/environment in relation to effective communication.
- Participates in appropriate multi/interdisciplinary team meetings.
- Documents patient’s weekly progress and goals set for next week.
- Reports status on each patient and sets new goals.
- Provides inpatient/outpatient diagnostic evaluations, consultations and follow-up therapy with patients.
- Exercises independent professional judgment.
- Administers appropriate diagnostic tools/instruments to diagnose disorders.
- Demonstrates knowledge/expertise of aphasia, dysarthria, voice disorders (organic), head trauma disorders, dysphagia in adult and pediatric populations.
- Provides resources to the patient regarding communication/swallowing disorders.
- Maintains knowledge of community resources available to communicatively impaired individuals to promote effective continuity and comprehensive care for outpatient or inpatient care.
- Appropriately follows policies/procedures regarding documentation, treatment and progress as recommended by ASHA and established by TAH.
- Documents patient’s evaluation, progress and discharge note on the patient chart.
- Provides evaluations on all patients – charts to include the date, case history, primary diagnosis, tests administered, impressions, goals, recommendations and signature.
- Performs other duties as assigned.
Education/Experience
- Master’s Degree in Speech Pathology with Certificate of Clinical Competency (C.C.C.) awarded by the American Speech-Language Hearing Association is required.
- Current license in Speech Pathology for the Commonwealth of Pennsylvania.
- One to two years of clinical experience as a certified professional preferred.
- Obtain FIM (Functional Independence Measure) certification within first year of employment and recertification every two years.
- Current BLS Healthcare Provider certification
- Stroke education is required
Knowledge, Skills and Abilities
- Required to maintain absolute confidentiality of patient care, patient accounts and hospital related matters
- Required to abide by, and comply with, the provisions of the TAH Corporate Compliance Policy
- Required to utilize positive Guest Relations principles in all interactions with patients, families, peers, third party payers and all members of the healthcare team
Pay: $38.00 - $42.00 per hour
