Registered Nurse – Care Continuum & Chronic Care Coordinator

Our practice is seeking an experienced, compassionate Registered Nurse who is passionate about delivering exceptional patient care across each stage of their healthcare journey within our facility. This is more than a traditional clinic nursing role. You'll become a trusted partner to both patients and providers, playing a vital role in surgical care, chronic care management, and day-to-day clinical operations.

1. What You'll Do Surgical Care (Pre-Operative, Intraoperative & Post-Operative)
- Prepare patients for in-office surgical procedures, ensuring all pre-operative requirements are completed and patients are educated, informed, and ready for surgery.
- Assist physicians during surgical procedures by maintaining a sterile field, anticipating provider needs, handling instruments and supplies, and ensuring patient safety throughout the procedure.
- Monitor patients during procedures and recognize changes requiring immediate intervention.
- Perform post-operative assessments, wound care, dressing changes, and patient education to promote optimal healing.
- Conduct post-operative follow-up calls and clinical evaluations to monitor recovery and identify complications early.
- Reinforce discharge instructions, medication management, activity restrictions, and recovery expectations.
- Coordinate surgical scheduling, pre-operative testing, referrals, and follow-up appointments to ensure seamless continuity of care.
2. Chronic Care Management
- Build lasting relationships with patients living with chronic medical conditions.
- Manage Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs through monthly patient outreach and ongoing care coordination.
- Monitor patient progress and identify changes requiring provider intervention.
- Develop and update individualized care plans in collaboration with physicians and advanced practice providers
- Educate patients on disease management, medications, preventive care, nutrition, and lifestyle modifications.
- Coordinate referrals, specialty care, and community resources to improve health outcomes.
- Accurately document all care management activities in the electronic medical record.
3. Clinic Responsibilities
- Triage patient concerns by phone and in person, providing timely clinical guidance.
- Room patients, obtain vital signs, update medical histories, and prepare patients for evaluation.
- Assist providers with examinations, procedures, and treatments.
- Administer medications, injections, immunizations, and other ordered therapies.
- Perform clinical assessments, specimen collection, wound care, and other nursing procedures within scope of practice.
- Maintain accurate electronic medical records and ensure timely documentation.
- Collaborate with physicians, advanced practice providers, medical assistants, and administrative staff to provide coordinated, patient-centered care.
- Support quality improvement initiatives, preventive health programs, and regulatory compliance.
- Deliver compassionate, professional care while creating an exceptional patient experience.
Qualifications
- Current Registered Nurse (RN) license in good standing.
- BLS certification required; ACLS preferred.
- Previous experience in surgical nursing, ambulatory surgery, primary care, chronic care management, orthopedics, cardiology, internal medicine, or family medicine is preferred.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and patient education abilities.
- Ability to prioritize multiple responsibilities in a fast-paced environment.
- Proficiency with electronic medical records and healthcare technology.
- A collaborative, positive attitude with a commitment to outstanding patient care.
