Overview
Cornerstone Caregiving is seeking a compassionate and detail-oriented Nurse Coordinator to oversee client assessments in a non-medical home care setting. This role involves conducting monthly evaluations of clients to ensure their needs are met, coordinating with caregivers and families, and maintaining accurate documentation of care plans. This position is very flexible on the schedule (weekdays and/or weekends), so long as all required tasks and patients are seen on a monthly basis.
Duties
- Perform client assessments to evaluate needs and care requirements.
- Sign off on Client Care Plans.
- Develop and assign client care plans to ensure quality care.
- Following up with clients in their homes and/or by phone.
- Completing and/or assisting with caregiver surveys, training and orientations.
- Complete weekly in-office skills checks.
- Providing input to improve overall quality of care provided.
- Complete monthly assessments on clients.
- Complete all notes and paperwork in the company platform.

- Must have a valid and non-expired RN license, and in good standings.
- Self-disciplined and self-motivated. It will be your responsibility to maintain routine of setting up schedule and completing monthly assignments.
As a Registered Nurse (RN), you will be essential in delivering high-quality patient care and ensuring optimal outcomes in the home care, non-medical, field. You will be reporting to and working with the Assistant Operating Director and other leaders. Strong communication skills and time management are crucial as you collaborate to deliver compassionate and effective care to our clients.
Pay: $32.00 - $33.00 per hour
