Position Overview:
We are seeking a dedicated and compassionate Developmental/Intellectual Disability (DD/ID) Care Coordinator to join our team in Covington County. This position operates within a Certified Community Behavioral Health Clinic (CCBHC) model, focusing on person-centered care, service coordination, and support for individuals with developmental and intellectual disabilities. In this role, you will conduct home visits, assess individual needs, develop plans of care, and help ensure access to services that promote independence, stability, and overall well-being.
Qualifications:

Bachelor’s Degree required in a mental health-related field

DD/ID Case Management training preferred

Strong communication, documentation, and organizational skills

Ability to work effectively with individuals with developmental and intellectual disabilities and their families

Knowledge of community resources, benefits coordination, and person-centered planning preferred

Ability to work independently and as part of a multidisciplinary team
Responsibilities:

Conduct home visits and gather relevant clinical, social, and environmental information from adults with developmental and intellectual disabilities

Develop and maintain individualized, person-centered plans of care based on each individual’s strengths, needs, and goals

Establish and maintain partnerships with community providers, medical professionals, support agencies, and other service systems

Assist individuals and families in accessing benefits, treatment services, housing supports, and other resources they are entitled to receive

Advocate for individuals and help reduce barriers to care and community integration

Collaborate with multidisciplinary team members to ensure coordinated, high-quality care within the CCBHC care model

Maintain accurate, timely, and confidential documentation in compliance with agency standards and CCBHC requirements

Support continuity of care and long-term wellness through ongoing follow-up and service coordination
ALL candidates must be able to type and have basic computer skills, possess a valid AL driver’s license, eligibility for coverage under center liability insurance and individually held vehicle liability insurance with a clear background check.
