This position will serve as a Care Coordinator for Lawrence while assisting with Olathe KSHOP participants in times of needed coverage as determined by supervision. The KSHOP program will support incarcerated residents with severe and persistent mental illness who also experience co-occurring substance abuse issues. The care coordinator will be part of a team identifying and screening participants, meet with identified participants pre and post release, advocate for appropriate programming and connect them with community-based resources; to include but not limited to housing, mental health and substance abuse treatment. This all will be accomplished by establishing partnerships with property managers/landlords, Bert Nash Mental health, Johnson County Mental health, substance abuse providers and other appropriate agencies. This position will work with providers, community groups and KDOC staff to identify gaps and develop strategies for filling those gaps. On occasion this position will work outside of traditional business hours to strategize and problem solve with the clients and staff. This position will also manage our current and future transitional properties.
This position supports case management of offenders under supervision with the Lawrence and Olathe Parole Office by providing housing resource navigation as outlined in program guidelines. This position will serve as the primary Care Coordinator for Lawrence participants and occasionally provide coverage for Olathe KSHOP participants as directed. The development and implementation of this program is new to Lawrence. Expectations of this position include creating strong community resource connections, and the cultivation of new, local partnerships to increase access to care for individuals supervised by the Lawrence Parole Office.
Serve as part of a liaison team between KDOC, Bert Nash Mental Health and Johnson County Health Center (when applicable) – Identify incarcerated residents with severe mental illness, drug and alcohol use disorders, and periods of homelessness who will be returning to Douglas or Johnson county areas and meet the KSHOP criteria. Meet with offenders prior to release to assess service needs either in person or telecommunication. Work with facility staff to ensure SOAR/RADAC screenings (and any other screenings necessary) are completed prior to release. Establish a method for accessing case information, as needed, for eligibility determination and case management while maintaining proper confidentiality.
Assess and identify housing needs for participants. Coordinate with KDOC parole and housing contacts to assist in locating safe and permanent housing for participants. Be available to landlords if housing placement is not successful or if issues arise that require intervention by this person with the participant or landlord. Ensure the apartment is ready for habitation (sleeping, preparing meals, showering, etc.) prior to participant moving in, by working with the Housing Specialists and local resources to address and fulfill needs.
Immediately upon release, connect participants to community-based resources, based on their identified needs and/or goals. This may include but not be limited to mental health, substance abuse treatment, employment services, etc. Remove barriers such as transportation that may limit service provision. Provide on-going one-on-one case management for participants who are not receiving case management services through Bert Nash Mental Health or Johnson County Mental Health Center (when directed) either because the service has not yet been started or because they do not qualify for Medicaid. Responsibilities may include securing permanent housing, connecting to community-based services, conducting house visits, conducting a weekly group meeting with participants, etc. Record interactions with clients and collateral contacts in the Athena database and maintain monthly participant engagement reports.
Facilitate multi-disciplinary meetings with community partners. This will include staffing program referrals with Bert Nash Mental Health and Johnson County Mental Health Center (when applicable) to ensure the participants meet the necessary criteria, staffing cases as needed when challenges or concerns arise, and providing programmatic updates to agency administrators monthly.
Meet regularly with supervisor to ensure needs of both Lawrence and Olathe programs are being met, training is up to date, and goals are being met.
Oversee transitional housing units and the memorandum of agreement for these units (or future units in Douglas County), as directed by the supervisor. These duties can include, but not limited to, placement, removal, maintenance, behavioral and contractual concerns. While also engaging with Johnson County's landlord recruiter to locate potential placements for individuals on parole.
Other duties as assigned to include but not limited to: Presentations for stakeholders and training/working with mentors who will deliver services to parole offenders, mental health training, education for staff, data and reporting
Six months of experience in planning, implementing and monitoring activities relevant to the agency’s programs.
Education may be substituted for experience as determined relevant by the agency.
Bachelor’s degree (B.A.) from four-year college or university in a human services or social science area and one year of experience in planning, implementing and monitoring activities relevant to the agency's programs
Masters degree with major coursework in criminal justice, the social sciences, or the behavioral sciences.
1 year experience working with landlords or an at-risk population related to housing.
Experience working with volunteers, especially mentors.
Experience recruiting support for an effort or initiative.
Demonstrated ability to engage, encourage and motivate.
Experience working with a multi-discipline team, including staffing cases.
Experience working with high risk/special needs population.
Experience in Community advocacy
