Speech-Language Pathologist (SLP) – Skilled Nursing Facility
Position Summary
We are seeking a skilled Speech-Language Pathologist (SLP) to provide skilled therapy services within our skilled nursing facility. The Speech-Language Pathologist will evaluate and treat residents with communication, cognitive-linguistic, and swallowing impairments and work collaboratively with the interdisciplinary team to support safe and effective resident care.
The ideal candidate will have strong clinical skills, sound clinical judgment, and experience working with the geriatric population.

Responsibilities
- Complete comprehensive speech, language, cognitive, and swallowing evaluations as clinically indicated.
- Develop and implement individualized plans of care based on evaluation findings and resident needs.
- Provide skilled treatment for communication, cognitive-linguistic, and swallowing disorders.
- Assess swallowing function and provide appropriate diet, positioning, and compensatory strategy recommendations.
- Monitor resident progress and modify treatment plans as clinically appropriate.
- Complete evaluations, daily treatment notes, progress notes, recertifications, and discharge documentation accurately and timely.
- Collaborate with Nursing, Dietary, Physicians, Rehabilitation, and other members of the interdisciplinary team.
- Educate residents, families, caregivers, and facility staff regarding recommendations, strategies, and safety precautions as appropriate.
- Participate in care planning and communicate changes in resident status or recommendations to the appropriate disciplines.
- Ensure services provided are clinically appropriate and meet applicable documentation and payor requirements.
- Maintain compliance with facility policies, HIPAA requirements, and applicable state and federal regulations.
Qualifications
- Current New York State license as a Speech-Language Pathologist required.
- Graduate of an accredited Speech-Language Pathology program.
- Certificate of Clinical Competence (CCC-SLP)
- Skilled nursing and/or geriatric experience required
- Knowledge of dysphagia management, cognitive-communication disorders, and speech/language interventions.
- Experience with Net Health and SigmaCare/Visual preferred.
- Ability to work independently while collaborating effectively with the interdisciplinary team.
Pay: $58.00 - $64.00 per hour
