Position Summary
Town Square Hospice is seeking a compassionate, skilled, and patient-centered Registered Nurse (RN) to join our team as a Weekend Nurse. The Weekend RN is responsible for providing high-quality hospice care to patients and families during weekends, ensuring continuity of care, symptom management, patient and caregiver education, and support during end-of-life care. This role requires excellent clinical judgment, strong communication skills, and a commitment to delivering exceptional hospice services with dignity, compassion, and respect.

Essential Duties and Responsibilities for Patient Care:
- Perform comprehensive nursing assessments and ongoing evaluations of hospice patients.
- Develop, implement, and revise individualized plans of care in collaboration with the interdisciplinary team.
- Manage pain and symptoms according to hospice standards and physician orders.
- Provide skilled nursing interventions and treatments as needed.
- Monitor changes in patient condition and communicate findings appropriately.
- Respond to urgent patient needs, symptom crises, and family concerns.
Family and Caregiver Support:
- Educate patients and caregivers regarding disease progression, medications, symptom management, and end-of-life expectations.
- Provide emotional support and guidance to patients and families during the hospice journey.
- Assist families in understanding hospice services and available resources.
Communication and Coordination:
- Collaborate with physicians, nurse practitioners, social workers, chaplains, hospice aides, and other members of the interdisciplinary team.
- Communicate patient updates and significant changes to appropriate team members.
- Participate in care coordination and handoff communication to weekday staff.
Documentation:
- Complete timely, accurate, and compliant documentation within the electronic medical record.
- Document assessments, interventions, patient responses, and communication according to Medicare Conditions of Participation and agency policies.
- Ensure all required weekend visit documentation is completed by the end of each shift.
On-Call and Weekend Coverage:
- Provide routine visits, PRN visits, admissions, and death visits as assigned during weekends.
- Participate in weekend on-call coverage as scheduled.
- Respond promptly to patient and family needs while maintaining professionalism and compassion.
Qualifications:
- Graduate of an accredited school of nursing.
- Current Illinois Registered Nurse (RN) license in good standing.
Experience:
- Minimum of one year nursing experience required.
- Hospice, home health, oncology, geriatrics, long-term care, or palliative care experience preferred.
- Strong assessment and critical thinking skills.
Certifications:
- Current CPR certification required.
- Valid driver's license, reliable transportation, and automobile insurance required.
- Valid RN license in the State of Illinois

- Understanding of hospice philosophy and end-of-life care principles.
- Excellent communication and interpersonal skills.
- Ability to work independently and make sound clinical decisions.
- Strong organizational and time-management skills.
- Compassionate, empathetic, and patient-focused approach to care.
- Ability to navigate electronic medical records efficiently.
Physical Requirements:
- Ability to frequently stand, walk, bend, lift, and transfer patients as needed.
- Ability to travel throughout the service area.
- Ability to lift up to 50 pounds with or without reasonable accommodation.

Pay: $40.00 - $42.00 per hour
