We are seeking a Home Health RN and LPN to join our team!
You will be responsible for the assessment, diagnosis, and treatment of assigned patients.
Responsibilities:
- Administer nursing care to ill, injured, or disabled patients
- Diagnose and establish patient treatment plans
- Monitor and report changes in patient symptoms or behavior
- Communicate with collaborating physicians or specialists regarding patient care
- Educate patients about health maintenance and disease prevention
- Facilitate referrals to other healthcare professionals and medical facilities
- Maintain accurate patient medical records
- Provide advice and emotional support to patients and their family members
Qualifications:
- Previous experience in nursing or other medical fields
- Ability to build rapport with patients
- Strong problem-solving and critical thinking skills
- Ability to thrive in a fast-paced environment
Looking for staff to cover Casa Grande and surrounding areas, Queen Creek, Apache Juntion, San Tan Valley, Picacho, Red Rock, and Marana, and Tucson
Staff are assigned to specific areas to minimize drive time.

Home Care Physical Therapist (Home Care PT) Responsibilities & Duties
Develop and implement individualized treatment plans for patients
Evaluate patients' progress and modify treatment plans as necessary
Educate patients and their families about their conditions and the rehabilitation process
Communicate with other members of the healthcare team to coordinate patient care
Document patient information, treatment plans, and progress in a timely manner
Maintain professional and technical knowledge by attending continuing education courses and workshops
Home Care Physical Therapist (Home Care PT) Qualifications & Skills
Experience in home health care or a similar setting.
Advanced certifications in physical therapy specialties.
Strong interpersonal and communication skills.
Proficiency with electronic medical record systems.
Ability to work independently and manage a flexible schedule.
Multilingual capabilities are a plus.
Bachelor's or Master's degree in Physical Therapy from an accredited program
Current state license to practice Physical Therapy
Experience in home health or geriatric care preferred
Excellent communication and interpersonal skills
Ability to work independently and as part of a team
Compassionate and empathetic approach to patient care
