About the Role
We are seeking an experienced and dynamic Clinical Supervisor to join our Home Health leadership team.
The Clinical Manager plays a vital role in overseeing day-to-day field operations, supporting clinicians, and ensuring every patient receives the highest level of care. This position works closely with the Director of Patient Care Services (DPCS) and other leaders to implement clinical best practices, maintain regulatory compliance, and cultivate a high-performing, compassionate team culture.

Key Responsibilities
Patient Care Quality
- Ensure all field staff understand and follow clinical processes and standards of care.
- Review and approve schedule requests based on patient needs and care recommendations.
- Conduct ride-alongs and provide constructive clinical feedback to field staff.
- Maintain high patient satisfaction through excellent clinical outcomes and service experience.
Field Team Leadership
- Support, mentor, and manage field clinicians to maintain compliance with agency policies.
- Ensure timely and accurate charting (within 24 hours of each visit).
- Conduct semi-annual performance assessments and ongoing education.
- Lead case conferences and coordinate staff training under the direction of the DPCS.
- Respond promptly to patient and family inquiries or concerns.
- Maintain strong communication and collaboration across the clinical and administrative teams.
Operational & Clinical Execution
- Implement clinical and staffing plans that meet patient needs and align with agency policies.
- Collaborate with scheduling, intake, and leadership teams to ensure efficient patient care coordination.
- Support the integration of community and contracted partners to enhance patient outcomes.
Leadership & QAPI Involvement
- Serve as a core member of the clinical leadership team.
- Collaborate with the Director of Clinical Services, Executive Director, and Scheduler to identify and address field team needs.
- Support ongoing Quality Assessment and Performance Improvement (QAPI) initiatives.
- Supervise and collaborate with the Clinical Administrative team to ensure timely documentation, coordination, and communication.
Qualifications
- Current Registered Nurse (RN) license in the state(s) of practice.
- Minimum 3–5 years of clinical experience, with at least 2-3 years in home health leadership preferred.
- Strong knowledge of Medicare Conditions of Participation and home health regulations.
- Proven ability to manage, educate, and inspire clinical teams.
- Excellent organizational, communication, and problem-solving skills.
- Ability to work collaboratively across disciplines and lead with integrity.

Job Type: Full-time
Pay: $110,000.00 - $135,000.00 per year
