Provide professional skilled care to patients in their home setting
Perform initial evaluation to establish goals, timeline and frequency
Develop a plan of care and manage care team
Perform discharge visit and determine whether goals have been met or care should be extended
Patient assignments made to a specific area in order to minimize travel
Document electronically
Ability to work independently
Maintain current CPR certification
