Social Service Designee
Job Summary:

Responsibilities:
Conduct initial assessments and develop personalized care plans to meet the social and emotional needs of residents
Provide counseling and support to residents, their families, and care staff to address concerns, needs, and concerns
Collaborate with interdisciplinary teams to develop and implement resident-centered care plans
Maintain accurate and detailed records of resident interactions, assessments, and care plans
Provide education and guidance to care staff on social and emotional support strategies
Participate in staff training and development programs to enhance knowledge and skills
Requirements:
1 year of experience in social work, counseling, or a related field
High school diploma or equivalent required; will train the right candidate
Possession of current professional licensure/registration as a Social Worker (LSW/LCSW) or a related field (RSA, RN, etc.) (if applicable)
Excellent communication, interpersonal, and organizational skills
Ability to work effectively in a fast-paced environment with multiple priorities
Proficiency in electronic medical records (EMRs) and other software applications
