Licensed Practical Nurse Hospice Case Manager

Responsibilities for this role include:
- Provide skilled nursing care to hospice patients within scope of practice and as
- delegated by the supervising RN.
- Perform nursing interventions such as medication administration, wound care,
- catheter care, vital signs monitoring, and symptom observation.
- Assist with pain and symptom management per the plan of care and physician orders.
- Observe, document, and promptly report changes in patient condition to the supervising RN.regarding patient needs and responses to care.(EMR) in accordance with organizational policy.Conditions of Participation (42 CFR Part 418), state regulations, and hospicepolicies.policies.2Provide skilled nursing care to hospice patients within scope of practice and asdelegated by the supervising RN.
- Deliver care in accordance with the individualized hospice plan of care.
- Communicate effectively with the RN case manager and other IDG members
- Participate in IDG meetings, in-services, and training as assigned.
- Complete timely, accurate clinical documentation in the electronic medical record
- Maintain compliance with Centers for Medicare & Medicaid Services (CMS)
- Maintain patient and family confidentiality in accordance with HIPAA.
- Practice nursing care within the scope of LPN/LVN licensure and organizational
- Follow infection control, safety, and emergency procedures.
- Always maintain professional boundaries and ethical conduct.
- Participate in quality improvement and survey readiness activities as assigned.

Pay: $67,500.00 - $78,000.00 per year
