Key Responsibilities
- Evaluate residents and develop individualized treatment plans.
- Provide skilled occupational therapy services to improve activities of daily living (ADLs), upper extremity function, cognition, and overall independence.
- Assess patient progress and modify treatment plans as needed.
- Collaborate with interdisciplinary team members to ensure optimal patient outcomes.
- Educate patients, families, and caregivers on treatment goals, home exercise programs, adaptive equipment, and safety techniques.
- Maintain accurate and timely documentation in compliance with facility and regulatory standards.
- Participate in care planning and discharge planning activities.
Qualifications
- Active California Occupational Therapist (OT) License.
- Graduate of an accredited Occupational Therapy program.
- SNF experience preferred, but not required.
- Strong clinical, communication, and documentation skills.
- Ability to work effectively in a patient-centered, team-oriented environment.
