Job Summary
We are seeking a compassionate and dedicated Hospice Nurse to join our team. The ideal candidate will provide high-quality care to patients in a hospice setting, ensuring comfort and dignity during their end-of-life journey. This is a PRN position for Crosby, Kingwood, and Humble areas.
Responsibilities
- Deliver comprehensive nursing care to hospice patients, focusing on comfort and symptom management.
- Conduct thorough assessments of patients’ physical and emotional needs.
- Develop and implement individualized care plans in collaboration with the healthcare team.
- Provide education and support to patients and their families regarding end-of-life care options and resources.
- Administer medications, treatments, and procedures as prescribed while monitoring for effectiveness and side effects.
- Assist with discharge planning to ensure a smooth transition for patients moving into hospice care.
- Maintain accurate documentation of patient progress, interventions, and outcomes in compliance with regulatory standards.
- Participate in interdisciplinary team meetings to discuss patient care strategies and updates.
Experience
- Previous experience in nursing, particularly in hospice.
- Strong communication skills to effectively interact with patients, families, and healthcare professionals.
- Ability to work independently while also being a collaborative team member in providing holistic patient care.
