Seagate Rehabilitation and Nursing Center is seeking a compassionate, creative, and experienced Activities Director to lead our Therapeutic Recreation Department. Plan, organize, implement, and evaluate a comprehensive therapeutic recreation and activities program in accordance with 42 CFR §483.24 and New York State regulations.
Develop individualized activity programs based on each resident's interests, abilities, needs, and care plan.
Create meaningful daily programming for residents with varying cognitive and physical abilities, including specialized dementia programming.
Supervise, schedule, mentor, and evaluate activity staff and volunteers.
Coordinate special events, entertainment, holiday celebrations, religious services, community outings, and intergenerational programs.
Complete required resident assessments, documentation, progress notes, and activity care plan updates.
Maintain monthly activity calendars and communicate programming to residents and families.
Collaborate with Nursing, Social Services, Rehabilitation, Dietary, and interdisciplinary teams to enhance resident quality of life.
Manage departmental supplies, equipment, and budget.
Ensure compliance with infection prevention practices, resident rights, life safety standards, and all federal and state regulations.
Participate in QAPI initiatives, interdisciplinary meetings, state surveys, and regulatory inspections.
Foster resident dignity, independence, engagement, and psychosocial well-being through innovative recreational programming.
Bachelor's degree in Recreation Therapy, Therapeutic Recreation, Recreation Administration, Gerontology, Human Services, or a related field preferred.
Current Certified Therapeutic Recreation Specialist (CTRS) certification preferred, or eligibility to obtain certification.
Must meet the federal requirements for a qualified Activities Director under 42 CFR §483.24.
Minimum of 2-3 years of activities or therapeutic recreation experience in a skilled nursing facility, rehabilitation center, assisted living, or long-term care setting.
At least 1 year of supervisory or management experience preferred.
Strong leadership, organizational, communication, and documentation skills.
